The Perinton Town Clerk's Office is a designated Passport Application Processing Center.  Applications for passports are processed by appointment only, Monday through Friday between the hours of 10:00am-3:30pm.  Requirements and fees can be found below.  Necessary forms and further information may be obtained in our office or online at http://travel.state.gov/passport/.  Please contact us at (585)223-0770 to schedule an appointment.

REQUIREMENTS:

ADULT PASSPORT (Age 16 and older)

·         Completed DS-11 passport application -  not signed
·         One 2x2 picture with white background (see Fees below)
·         Birth Certificate with raised seal and both parents names
·         Driver's License
·         Previous Passport if applicable (call for more information)
·         Social Security card (not required)

MINOR PASSPORT (Age 15 and younger)

·         Child must appear in person
·         Both parents (or guardians) must be present
·         Completed DS-11 passport application – not signed
·         One 2x2 picture with white background (see Fees below)
·         Birth Certificate with raised seal and both parents names
·          Both parents driver's license or valid passports as proof of Identification
·         Student ID and/or social security card (not required)

PASSPORT BOOK: 4-6 weeks to process
